Confidentiality and Privacy
I will always maintain your confidentiality and will not share your information with other health providers (or any third party) unless requested in writing by yourself. Nourish Your Nutrition uses a secure, web-based practice management system to store and manage client records. This includes information such as appointments, billing documents, session notes, contact details, and other relevant information and documents. The system is encrypted and has servers exclusively located in Canada; access to the system is password protected and limited to myself. Paper files are kept under lock and key and will be destroyed after seven years.
It is the policy that I am not able to accept social networking invitations (i.e., Facebook, Snap Chat, Instagram etc.) from past or current clients on my personal accounts. It is acceptable to accept social media invitations on professional accounts ( ie. nourish your nutritions Facebook and Instagram).
Our work together may take place in a traditional office setting, or, on nice days, we may opt to go for a walk or sit outside. I will also be offering virtual counselling on an easy to use platform for your convenience.
Insurance and Coverage
My services may be covered by your insurance provider. Please contact your provider to verify whether the services of an RD are covered. Direct billing is only available with Blue Cross. As I do not direct bill for other insurance providers, payment is due up front and you will be provided a receipt to submit to your provider or for use on your tax return. I am not registered with your Employee/Family Assistance Plan.
Fees and Payment
Initial assessment: individual, family, and couple sessions are 60 + minutes in length and are billed at the following rates $150
Follow up sessions:
30 minute = $75
15-minute check-in $37
Nourish your nutrition requests that payments be made at the end of your session. Methods of payment accepted at this time are e-transfer or cheque. E-transfers can be sent to email@example.com. If a client fails to pay within 30 days of their appointment they will be discharged from Nourish. Packages can be worked out based on clients needs and a discounted rate will be applicable.
